Problems with IK

I've recently started working with Duik. Since this time i had only some small problems i was able to fix by my self.
Now, i have no idea what to do, please help me! ( As i think, it will be a small bug, and i just don't see the problem )

While rigging legs i had same problem. I solved it, by multiplying rigged Layers+Controler, but i guess its not how it should be done.
Im trying to rig a hand. I am using anchorpoints, no muppets

I did Created controler, and placed it on propper position, then choosed layers, forearm/arm/controles, then pressed ik, and choosed 2layers ik. My controler stayed in the same position, but hand i was truing to rig, twisted in some wierd way :/

Does any one could help me with some solution?
